I think if you are not married you can put the fathers name on the Birth Certificate. Or leave it as yours.. I know if you are married they just about always put the husbands last name on it... Unless told otherwise, never seen that happen before though.

I live in Wisconsin (in the US obviously) but my son was born in Michigan (my daughter will be, also) since we live right on the border.

Anyways, they asked me at the hospital who's last name my son was going to have. And since I've had to go into the OB unit twice they've got it all set up so this baby will have my last name, too. However, that is my and my OH's decision to give the kids my last name and it would have been just as simple for us to give Aiden (and this baby) his last name. They ask and you fill out the paperwork for the birth certificate. It's really simple.
