Got my questionaire


Wow that was fast. I got my Questionaire in the mail already.... So now I need to fill it out and send it back... Here is what they said:

After you return the questionaire and we rreceive any previous medical records, you will receive a phone call to schedule your first fisit at our informational fertility seminar. You will receive instructions to begin your evaluation, which typically includes a semen analysis, blood work and a hysterosalpingogram (also called HSG, and X-ray of the uterus and fallopian tubes). We will schedule your workup appointment with a fertility provider to decide the specific treatment that best suits your individual situation after the initial tests are completed.

Now I need to figure out what this will cost us. It is with my heath insurance, but I need to research what is and is not covered. But im sure it will be all worth it.

i know insurance is tricky i know mine doesn't cover any infertlity thing and i thought i had good insurance
already i have to do a ultrasound if i am not pg this month and i haven't met my $2000 detectuble it will cost me $700
so i said look into it for sure that you can plan finances
