The motility is low and the shape isn't good but there is still a chance.
After 3 months if those 2 things are still low, get a referal to a fertility specialist. They will want to check you out fully to make sure there is nothing wrong with you and they'll want to do a proper sperm test (wash, spin etc,) to investigate that further.
If the sperm doesn't change in 3 months and everything is ok with you, i'm *guessing* they will want to do IUI or something, if that doesn't work there is IVF.
Fingers crossed that 3 months shows a rapid improvement
My DH's motility has ranged on 3 tests from 1-30% and all my investigative surgery checked out fine. We've been trying for 18 months and have been unable to fall pregnant in that time, but his motility is much lower than you DH's. We're waiting on the scientific sperm wash and depending on those results the specialist said we'll either be doing IUI, IVF or ICSI. Just to give you a small idea.
my dbf was motility 6% and our consultant didnt seem too concernec. said it was borderline and worth a go with clomid for 6 months then onto ivf. but his count s only 23million. he has been taking lots of vits and will be retested 6th may three months sinc last test
