birth plan

birth plan, im not sure how it works or what you write on it.
just wondered if any one could tell me or has a spare copy or a draft copy?

Do you not have a blank sheet in your maternity notes that has questions on it for you to answer.....??? thats what i did for my own...just filled out that

I've just been told to write down my wishes in order of preferences.

i liked it as it didnt need to much writing and covers everything you need
