Who needs to sign your Sure Start form?

Well I wasn't planning on claiming it this soon but my DH has just come home and said his clutch has gone on his car so that's us completely buggered for money

If I made a doctors appointment could I get just a normal GP to sign it? What other info do I need before I can send it off? I haven't got the form yet, I'm going to pick one up from the job centre tomorrow so just want to be prepared so I can get it sent off asap. I'm a bit gutted that all the money will have to go on DH's car but that's what cars do I suppose... break down when you need them the most

yer you can get your gp to sign it or your health visitor and mw! as long as they are a health proff.
you dont need to send anything off with it i dont think, just as long as you know all your info like what money you get ect

yeah your doc or midwife can sign it they are bang on it and youll get your money within 5 days of sending it off i always have

My midwife signed mine and i sent it away weeks ago, i got it returned last week coz she hadnt stamped it... So make sure whoever signs it remembers to stamp it also !
