I am not doing one this time, but with my first just did more of a bullet point list.ie
*I would like OH to cut the cord
*I would like to use the pool if possible
etc etc.
Was told this was more likely to be followed as its easier to read.HTH.xx

I'm writing mine as I type this, and I fear it will turn into an essay.

I'm using examples from www.thegoodbirth.co.uk and www.homebirth.org.uk as I'm planning a homebirth and changing and adding bits

As for hospital births, you can google birth plan template or birth plan examples and it comes up with loads

My mw gave me a questionnaire thingy that we just had to tick the answers against then add any other info we wanted.

Questions in it are
Who do you want as birth partner and do you want him/her there during delivery?
During labour would you prefer walking, resting in bed or wait and see?
What kind of monitoring would you prefer?
What methods of pain relief would you prefer?
What labour position do you think you'd prefer?
Do you have any objections to vaginal examinations?
Do you have any objections to your waters being broken?
Do you have any objections to Syntometrine injection being given?
Do you have any objections to the baby's cord being cut at delivery?
Do you have any objections to the injection of Konakion being given to the baby at delivery?
Do you have any objections to students being present?

I downloaded and changed one to suit me and my OH.

Preferences for Labour and Birth for Charlotte Lee
Labour

I expect that doctors and hospital staff will discuss all procedures with me before they are performed.
I would like to be free to walk, change positions and use the bathroom as needed or desired.
I prefer to eat and drink throughout labour, as desired.
I will remain hydrated by drinking moderate amounts of fluids (water, juice, ice).
Please limit the number of vaginal exams.
Please allow me to vocalize as desired during labour and birth without comment or criticism.
Please do not permit observers such as interns, students or unnecessary staff into the room without my permission.
Anaesthesia/Pain Medication
Please do not offer anaesthesia unless I ask for it.
If pain relief is considered, I would like to try a narcotic before an epidural.
Caesarean Section Delivery
If a caesarean is necessary, I expect to be fully informed of all procedures and actively participate in decision-making.
I would like Bill Yarde to be present during the surgery.
Please explain the surgery to me as it happens.
I would prefer general anaesthesia in an emergency only.
I would prefer epidural anaesthesia, if possible, in order to remain conscious through the delivery.
If possible, please do not strap my arms to the table during the procedure.
If conditions permit, our baby should be given to Bill Yarde immediately after the birth.
I would like our plans outlined here for after the birth to be followed as closely as possible.
Please lower the screen just before delivery so I may see the birth of the baby.
Perineal Care
I prefer not to have an episiotomy unless it is medically indicated.
To help my perineum stretch, please help guide my pushing efforts by letting me know when to push and when to stop.
Please administer local anaesthesia when repairing any episiotomy or tear(s).
Delivery
I do not want to use stirrups while pushing.
I would like the freedom to push and deliver in any position I like.

Immediately after the birth
Bill Yarde would like the option to cut the cord.
Please remove my IV/Heparin lock/catheter as soon as possible after delivery.
Newborn Care
I would like to hold Our baby skin-to-skin during the first hours to help regulate baby's body temperature.
If our baby must go to the nursery for evaluation or medical treatment, Bill Yarde, or someone I designate, will accompany our baby at all times.
Postpartum Care
If available, I would prefer a private room.
I would like to have our baby room-in with me at all times.
Once I've had time to recover, I would like our baby to room-in with me.
I would like Bill Yarde to room-in with me.
Assuming I feel up to it and our baby is healthy, I would like to be released from the hospital as soon as possible following the birth.
I would like permission for access to my chart and our baby's chart.
Additional notes
I would like to take still photographs during labour and the birth.
