hmmm how on earth am i supposed to get my MatB1 form?? The hospital have run out, my doctors don't carry them and their sister practice has ran out aswell. Work are gettin moody about it, because they can't confirm my leave until they receive it. They also have to send it to payroll so they can return this form to me, then i can apply for MA. It's gonna take ages isn't it?! What makes it worse is that i'm taking maternity leave early really because i actually work out better off on MA.

Is anyone else having difficulty gettin their form? Anyone know anywhere else i can try to get 1?

My midwife had tonnes of them - they look like they photocopy blank ones to me. I dont understand what the problem is????? Tell them your work want it 15 weeks before you are due to leave and if you dont get your leave because of them you will not be happy......

The only way I got mine was to be pushy about it. Say you can't leave until you have one because it is causing you so much stress, guarantee you will leave with a MATB1 form.

I'm popping down to the MW drop in tomorrow with my blue notes and they said they can do one then and there.... Any GP or MW can do one, as long as they see your notes

Thats what I thought Ann-Marie. My m/w had a stack of them and asked me if I wanted it and just filled it in.

My MW wouldn't give me 1 till after 20weeks but i have it all sorted now! actually still sitting around the hse..... oops! best get it sent to uni! xx

I would keep phoning up and complaining - possibly go into your GPs about it. It is important that you get it in before a certain date. I kept getting told that they would not give them out until midwife appointment at 24 weeks, so luckily I got my one last week. Remember to get a copy for your OH as he will have to give it into his work to get his paternity leave.

Is that also the form we can use to get free prescriptions and things??? I've had to pay for my dentist visit recently so it would have been nice to have had that free.

I wont be getting that form until 20 weeks I've been told!???

No - the one for free precriptions you can have before you're 20 weeks - I applied for mine via the Dr's receptionist when I was just 7 weeks. I knew I was going to the dentist so I wanted in time for that !

The MATB1 entitles you to maternity leave/pay and thats the one they cant give you until after 20 weeks.
