I am writing to inform you that I have recently discovered that I am pregnant and will require to take maternity leave. My baby is due on*date* and I wish to take my leave from*date* to *date*
I have enclosed a copy of my due date for my midwife/GP *delete as applicable*
Please could you confirm these dates with me and if you would like any more information I will be happy to discuss with you.

I'm Currently Feeling: | lol if you go onto the directgov website threr is a letter on there that you can print off, it states everything you need to but also the dates you are entitled to leave, how long you are entitled and how much money you will receive..
