Dear *

I am writing to inform you that I have recently discovered that I am pregnant and will require to take maternity leave. My baby is due on*date* and I wish to take my leave from*date* to *date*

I have enclosed a copy of my due date for my midwife/GP *delete as applicable*

Please could you confirm these dates with me and if you would like any more information I will be happy to discuss with you.

Regards,
*

"Hi Guys

No doubt youve already heard but wanted to put it in writing that i am pregnant. EDD is 18th of December but as you know it can chop and change.

Let me know if theres anything you need me to do.

Thanks
Holly"
