Just call them up, and explain the situ they will guide you through everything.

The rate given is slightly dependent on your status, are you an employee of your company or sole trader, director etc,

Im technically a employee of my own ltd company, but have status as a director so i have control over my wages.
For me it went on my last years self assesment figures, they took that and divided by 52 to get my weekly wage.

If you not a director in any way you just have to prove the last 8 weeks i think, ie payslips and national insurance details etc.

You fill an online form in, its really easy, then they will reimburse the company either weekly or in a one off payment for the period of time you will be off. Plus a small handling amount.

Be aware if you have a good accountant he may have made it so that may have received a low wage (escaping so much tax to pay) but got more in way of bonuses dividends etc. This will bite you in the bum as then your actual wage seems really low when they work out you entitlement.
